Please bring with you, your TBA registration fee. We will be sizing the players for Colts t-shirts and pants as well as handing out helmets. If your son would prefer to use a padded girdle this season he may use one he owns or you can purchase one for $25 during registration night.

In preparation of Season Registration, players are required to provide the following: (the forms will be provided on registration night but the info will need to be handed in before ____)
CAMFA Required Medical Health Questionaire and the Consent & Release Forms
PROOF OF PLAYER’S PLACE OF RESIDENCE: copy of a Jan/Feb/Mar 2021 Utility Bill

Welcome to the 2021 season of the Colts Spring League Football Club!
We will be running 2 teams this year due to Covid, with a 50 cohort maximum per team.
Senior Team (Grade 11s and Grade 12s)
Junior Team ( Grade 10s and Grade9s)
If you are printing off these forms and filling them out at home we need hard copies. Please DO NOT email them back to us.
Please follow these instructions-
1- All the forms need to be filled out completely before your child is permitted to participate in any manner on the team.
2-Please provide a PHOTOCOPY of your child’s birth certificate, Alberta Health Card and your proof of address (ENMAX OR DIRECT ENERGY BILL-with your current address). We will not be taking pictures of cards for us to print off.
3-Please provide an Equipment deposit cheque for $350.00 dated June 1, 2020. These will not be cashed unless the equipment is returned in an unusable fashion (including unwashed.)
4-Please bring completed forms to the locker room WITH payment. We will have a Square for payments this year. VISA, MC, Debit, or AMEX. We do not accept cheques as payment. If you require any financial assistance please speak to Treena, Tracy, Trish, Amy
The CSFA league has really emphasized to us this year that all forms need to be filled out before any player can go on the field. Even during Spring Camp. Our team can be penalized if we don’t follow this. We appreciate your help and cooperation with this!
